This chapter describes the indications for maxillectomy reconstruction and the historical evolution of the various methods used to reconstruct the mid‐face. It discusses the pros and cons of the various defect classification systems. The ideal classification system of the maxillectomy defect should consider the following criteria: (a) dental status, (b) oro‐antral/nasal communication, (c) contiguous structure involvement, (d) superior–inferior extent, (e) anterior–posterior extent and (f) medial–lateral extent. In the absence of such ideal classification, this chapter uses the Cordeiro classification to show the various defects commonly encountered. It also outlines the pitfalls and the important steps that need to be taken into consideration to accomplish a safe reconstruction. Some illustrative cases are shown.
