Epididymal and testicular abscesses: A case report and review of the literature

Abstract

Epididymal and testicular abscesses are uncommon complications of epididymo-orchitis traditionally managed with open surgical incision and drainage or orchiectomy. We report a 75-year-old man with polymicrobial scrotal abscess presenting with dysuria, pain, swelling, and mild leukocytosis. Diagnosis was confirmed by scrotal ultrasound, and he was treated with ultrasound-guided percutaneous catheter drainage and antibiotics. Improvement in pain and swelling followed with resolution of leukocytosis. This case and our narrative review demonstrate that image-guided drainage is a feasible organ-preserving option for selected patients with accessible collections, although evidence remains limited to case reports and small series.
