Using Health-Related Quality of Life Assessments to Evaluate Care Support Within Medicaid
Abstract
CareOregon, an Oregon-based not-for-profit Medicaid health plan, successfully piloted a "CareSupport" model that identifies high-risk members and clinically stratifies them for intervention.
