Radiological Cases of the Month: Case 2

Abstract

A FULL-TERM infant weighing 3153 g was delivered by spontaneous vaginal delivery following an uncomplicated pregnancy. The Apgar scores were 8 and 8 at 1 and 5 minutes, respectively. On the second day, tachypnea (80 breaths per minute), subcostal retractions, and duskiness were noted. The heart rate was 140 beats per minute, and peripheral pulses were normally palpable. The blood pressure measurements in all extremities were normal for age and gestation, and the precordium was hyperactive. A grade 2/6 systolic murmur was heard over the lower left sternal border. The liver was enlarged, was palpable 3 cm below the right costal margin, and was not tender. A diagnosis of congestive heart failure was made. The laboratory findings were as follows: white blood cell count, 15.1×109/L with a normal differential cell count; hemoglobin, 148 g/L; and platelet count, 151×109/L.
