Applying the Bridging the Transition Framework

Abstract

Building on the population-level strategies for supporting function and participation in daily life presented in Chapter 8, this chapter explores examples of homelessness prevention and intervention at the population level by addressing structural factors through policy change, research, and advocacy. We highlight three key strategies to guide work at the population level: (1) identifying the problem, (2) building relationships, and (3) articulating a clear message. We then outline how these strategies supported efforts or created challenges in three different examples from our work in Ontario, Canada. These examples provide lessons learned for how to enact positive change at the population level utilizing the Bridging the Transition Framework. By reflecting on these experiences, we highlight the benefits and challenges of population-based work focused on addressing trauma-informed care, poverty prevention, and access to mental health and addiction services.
