The Integrated Health Hub (IHH) Model: The Evolution of a Community Based Primary Care and Mental Health Centre
Abstract
There is increasing demand for comprehensive community healthcare that integrates community mental health and primary care services. The Canadian Mental Health Association-Durham Branch provides an example of how the Integrated Health Hub (IHH) Model evolved organically to meet this need.
