Essential Medical Evaluation

Abstract

Geriatric has been defined as 65 years or older since 1935 when life expectancy in the United States was 58 years (Social Security Administration, Life expectancy for social security. Social Security History, 2023). Mean life expectancy in the United States has since increased, by some measures to 79 years. As a result, a larger proportion of the population now can be considered geriatric.Eighty six percent of individuals over 65 years have at least one chronic systemic medical illness, and more than 50% have at least two chronic conditions (National Center for Health Statistics, Percent of U.S. adults 55 and over with chronic conditions, 2015). Systemic medical illnesses in this age-group often include psychiatric symptomatology, such as depressive symptoms in cardiac disease, or psychotic symptoms in major neurocognitive disorder (MNCD). A thorough medical evaluation, therefore, is indicated for geriatric patients admitted to a long-term care (LTC), skilled nursing facility (SNF), or psychiatric unit, as well as for those presenting for outpatient psychiatric care.The reader is referred to many excellent textbooks of geriatric medicine and to online resources for up-to-date guidance in medical diagnosis and treatment.This chapter provides a brief overview of systemic medical conditions and medical events common to geriatric patients who are in a long-term care facility (LCF), Skilled Nursing Facility (SNF), psychiatric unit, or similar unit, as well as those seen in outpatient settings. It offers recommendations for prompt recognition and referral to specialty care or urgent medical care. The goal is to reduce systemic medical complications, avert prolonged stays, and minimize the severity and duration of the conditions and associated psychiatric/behavioral symptoms.
