Linear IgA dermatosis: An unusual case involving the oral mucosa and possible association with perindopril therapy

Abstract

Background: Linear IgA dermatosis (LAD) is an autoimmune cutaneous bullous disorder characterized by IgA antibodies directed towards various basement membrane antigens. A significant percentage of patients have mucosal involvement. Cicatricial pemphigoid is another bullous disorder with predilection for mucous membranes and is characterized by linear IgG and C3 deposits along the basement membrane zone. Occasionally, IgA is the only immunoreactant detected. Method: We report an elderly gentleman on perindopril who developed recurrent oral bullous lesions with IgA deposits detected along the basement membrane zone on direct immunofluorescence. Results: After perindopril discontinuation and treatment, the patient had no evidence of skin lesions or systemic involvement after one year of follow-up. Conclusion: These features are consistent with the diagnosis of LAD involving the oral mucosa alone which may have been related to perindopril therapy.
